We're not sure what the tectonic situation is like under the Mushroom Kingdom, but there is a lot of activity. For so many castles to have lava underground, we have to question the site selection of the builders. When it comes to playing Mario games, lava is a huge OHSA issue since not only is there lava on the floors, but lava balls flying around and even fireball lines that rotate.

Thwomp isn't the most dynamic of video game enemies. He typically sits in one spot, moving up and down. Most sidescrolling players should have no problem getting past this dunce. Sometimes, however, you're in a hurry, don't time your jump just right, and end up in a crash course about gravity.
While Thwomp's attacks can be chalked up to creative level design, we can't get over just how absolute dying from him is. It's not like you brushed a Goomba, or fell down a pit that alludes to the idea that you may have survived. Unless we're talking about a Gumby Mario, Thwomp's killing of you is pretty cut and dry.
2 𝐆 Falling Off The Slide In Cool, Cool Mountain
In what was considered a fantastic add-on to the level that is also a mini-game to get an extra star, Cool, Cool Mountain's ice slide is a killer of Marios. Since nobody decided to install guard rails on the slide, if you misjudge your speed and direction, you can be sent hurdling towards a bottomless pit like Luke Skywalker in Cloud City.
This all happens as cheery, up-tempo music plays in the background while Mario screams on his way down to the abyss.
