If you make the wrong decisions or mess up on some QTEs, you can quickly kill off some of your characters. None of thღe playable ones can die until chapter three, though. They're all fully-voiced characters who have their own skills and abilities, so it's easy to grow attached to them. That's what's so dangerous about having permadeath turned on because they can easily be killed during missions, especially if you generally avoid the stealthy approach. At least if you lose someone, there are streets filled with people who can take their place.

Throughout most of Mass Effect 2, you're recruiting different companions in an effort to build an incredible squad that can take on the Galaxy-wide threat posed by the Collectors and their rulers, The Reapers. Not only do you recruit these people, but you can also create a bond with them, get to know them on a deeper level, and, in some cases, even pursue a romance with them.
Then, in the final mission of the main story, the aptly titled Suicide Mission, some or all of them can die. You can significantly lower the chances of this happening by gaining your companions' loyalty, upgrading your ship, and making the right choices during the mission.
