Aside from consuming the perk and dying, there is no manual action that you need to take to use tombstone and save your rucksack items.

When you launch into your next game, a tombstone will automatically be marked on your map. Travel here to collect your items.

If you die and are brought back to life, the tombstone will not appear. Additionally, if you die with tombstone, come back to life, and then exfil, a tombstone will not appear. You must die and leave the game in order to spawn a tombstone stash marker.
